A peek under the hood

Here's how we land on your number. We match your insurance type with the medication you chose, then let your BMI decide whether weight-loss coverage applies (most plans ask for a BMI of 27 or higher before they'll cover a weight-management drug). Three sources feed the estimate:

  • Manufacturer list prices from Novo Nordisk and Eli Lilly.
  • The NovoCare and LillyDirect cash-pay programs for self-pay patients.
  • The CMS Medicare Bridge pricing planned for July 2026.

For commercial insurance, we assume you qualify for a manufacturer savings card that caps your copay near $25 a month. We treat compounded semaglutide as cash-pay, since insurance doesn't apply to it. Your real plan may differ, so use this as a starting point for the conversation with your insurer.

Questions we hear a lot

We built it on April 2026 manufacturer list prices, the NovoCare and LillyDirect cash-pay programs, and the CMS Medicare Bridge pricing planned for July 2026. That said, your true cost still hinges on your specific plan, your pharmacy, and how prior authorization shakes out — so think of this number as a helpful ballpark to plan around, not a quote.
That $998 is the manufacturer's list price for a one-month supply, and it can feel steep. The good news: if you're paying cash, the NovoCare self-pay program often brings it down, and most insured patients pay far less once their plan and any savings card are applied.
Starting July 1, 2026, CMS plans to let eligible Medicare beneficiaries fill covered weight-loss GLP-1s for a flat $50 a month. To qualify you'll generally need Part D or an MA-PD plan and to meet the clinical criteria. We'll walk you through it in our Medicare eligibility checker.
Usually, yes — telehealth compounding pharmacies tend to charge $99–199 a month all in. Compounded semaglutide is prepared by licensed pharmacies and is legal when the FDA recognizes a shortage. It's worth understanding the trade-offs, which we cover in our compounded GLP-1 guide.
The compounded estimate bundles in a typical consult, but the brand-name estimates show only the medication copay. Most telehealth providers add their own consult and shipping fees on top, and those vary from program to program, so it's worth checking the fine print before you commit.
Unfortunately, manufacturer copay savings cards almost always require commercial insurance and exclude government plans. If you're uninsured, you'll usually do better with the manufacturer cash-pay programs or a compounded option — both of which this calculator can show you.
