Why coverage looks so different across state lines

If it seems unfair that someone two states over pays nothing while you face full retail, you're not imagining it. Here's the reason — and what it means for you.

Medicaid is funded jointly by the federal government and the states, but each state runs its own program — and that includes deciding whether to cover weight-loss medications at all. Diabetes-labeled GLP-1s like Ozempic and Mounjaro are covered nearly everywhere. The weight-loss versions, Wegovy and Zepbound, are optional, which is exactly why coverage feels so uneven from one place to the next.

In the states that do cover them, you'll usually need a prior authorization plus a BMI of 30 or higher (or 27+ with a related condition like type 2 diabetes, high blood pressure, or sleep apnea). Partial-coverage states draw the lines tighter still — sometimes just a single covered drug, or a BMI bar set at 40+. None of it is a reflection on you; it's policy, and policy can change.

Here's a reassuring twist: no matter where your state's Medicaid lands, the Medicare GLP-1 Bridge gives Part D enrollees access to Wegovy and Zepbound for $50 a month starting July 1, 2026. It runs through December 2026, and then the BALANCE Model takes over in January 2027 — so there's no gap to worry about.
