A peek under the hood

We look at three things you tell us — your insurer, your medication, and why it's being prescribed — and weigh them against how coverage usually works. The big factor is the reason: diabetes medications (Ozempic, Mounjaro, Rybelsus) are far more likely to be covered when the diagnosis is type 2 diabetes, while weight-loss medications (Wegovy, Zepbound, Foundayo) typically need prior authorization and a BMI threshold — and can be left out entirely on plans without a weight-loss benefit.

Medicare and Medicaid play by their own rules. Medicare's weight-loss coverage opens up through the Bridge program in July 2026, and Medicaid coverage really does change from state to state, so we treat those paths separately.
